Minimally Invasive Dialysis Access at IVIRA
At Integrated Vascular & Interventional Radiology Associates of Pittsburgh (IVIRA), we specialize in the creation of Percutaneous Arteriovenous (AV) Fistulas—a groundbreaking, minimally invasive alternative to traditional surgical fistula creation. For patients in need of hemodialysis access, this advanced procedure offers quicker recovery, fewer complications, and a gentler experience overall.

What Is a Percutaneous AV Fistula?
A Percutaneous AV Fistula is an artificial connection between an artery and a vein, created using specialized endovascular tools—without large surgical incisions. This fistula provides the high blood flow needed for effective dialysis treatment.
Percutaneous AV fistulas are made using ultrasound and fluoroscopy guidance, typically in the arm. The two most common devices used are:
-
Ellipsys® Vascular Access System
-
WavelinQâ„¢ EndoAVF System
Why Choose Percutaneous AV Fistula Over Surgery?
-
No large incisions or sutures
-
Performed under local anesthesia
-
Less pain, faster healing
-
Outpatient procedure with same-day discharge
-
Lower risk of infection and stenosis
-
Earlier readiness for dialysis in some patients
Who Is a Candidate for Percutaneous AV Fistula?
Ideal candidates include patients who:
-
Require long-term hemodialysis access
-
Have suitable vessel anatomy for endovascular creation
-
Prefer a less invasive option compared to open surgery
-
Want to avoid hospitalization and general anesthesia
At IVIRA in Pittsburgh, we work closely with your nephrologist and dialysis care team to determine your eligibility through ultrasound vein mapping and pre-procedure imaging.
What to Expect During the Procedure
Using a tiny puncture in the arm and real-time imaging, our interventional radiologist guides catheters into place to connect an artery and a nearby vein. Once the connection is made, blood flow begins through the new AV fistula.
The entire procedure takes about 60–90 minutes, and you can go home the same day. Your new fistula may be ready for dialysis within a few weeks.
Follow-Up and Monitoring
At IVIRA, our care doesn’t end after the procedure. We offer:
-
Regular ultrasound evaluations
-
Maturation assessments
-
Interventions if the fistula shows signs of narrowing
Ongoing coordination with your dialysis provider
